Sports Teams at G.W. Carver HS

PSAL FORMS REQUIRED FOR PARTICIPATION

Before trying out for or joining a team, the following forms must be completed and submitted:  

Interscholastic Athletics Parental Consent Form

A completed form includes:

    • The top section completely filled out
    • initials in all 13 spaces
    • Emergency Contact Number
    • Parent Signature and date

 

Interscholastic Sports Medical Form 

Please submit Page 3 to your Coach. A completed form includes:

    • Student-Athlete's information on the top of the page
    • Medical Provider's Clearance
    • Required accommodations, allergies and/or medication indicated 
    • Medical Provider's information, stamp and registry number

Please note:

  • Please do not complete forms in pencil.
  • Completed forms should be handed directly to their coach.
  • Do not deliver, mail or fax forms to the school.
  • Student-Athletes must submit original versions of these forms
  • Scans, copies, faxes and emails will not be accepted
  • Alternate forms will not be accepted.

 

Please email your coach for tryout and practice schedules.

  • Fall Sports season starts in August before we return to school.
  • Winter Sports begins in November with tryouts held in October.
  • Spring Sports season runs from March to June.
